Submit Physical Form - Cambridge Middle School

 Physicals are good for three years and must say "cleared for sports. 

 You can get the blank physical form here.

  Need to check the date of your child's last physical? 

  Go to Skyward Family Access

  •  Click on the Health Info Tab on the left
  •  Next to the student name, choose "display options"
  •  Show physical exam

  Once your doctor completes the physical, turn it in to the activities office of your child's school; or scan and submit it electronically below.
